I've always considered fear of failure and fear of success to be two sides of the same coin. They're both rooted in self-sabotage and so the initial piece of "why" doesn't really matter too much. Self sabotage is difficult to identify, but also difficult to stop, and there are a lot of messages that we got from growing up that is embedded in this, so it can be challenging to break free from, but necessary to confront if you want to life a better life.
There are enough barriers and challenges in life without being one to our own selves.

Set Small, Achievable Goals: Break down your larger goals into smaller, manageable steps. This makes the path forward less daunting and more attainable.
Seek Support: Don’t hesitate to reach out for help. Talking to a mentor, coach, or therapist can provide valuable insights and encouragement.
By understanding the roots of self-sabotage and implementing strategies to stop doing it, you can move past these barriers and create a path toward greater success and fulfillment.
